Q: Does it work only when the turn signal is on?

It works all the time. If you turn your signal on and there is a vehicle coming up beside you it will be beep. Otherwise anytime a vehicle comes up beside you the light will flash yellow to let you know that a vehicle is beside you.

Q: Does the system provide cross traffic alerting when in reverse?

Yes. cross traffic alert works perfectly.

Q: will it work on lexus gx 470?

If the sensors will fit under the rear bumper cover the system should work. However, the car has to be a newer model because the device installs to the OBDII port. I am not sure of the year cutoff but you can contact Brandmotion direct and ask them.
