Q: does this hurt when first used ? i mean do you have to get used to wearing ?

You would need to get use to it. It dose not hurt but it depends how severe you problem is. It is a great product.

Not sure if it cures it but I have been using this for about 2 weeks and can notice a big difference

Q: How often and how long a day should I wear this?

I try to keep it on for 1/2 hour per day. You can't wear your shoes with it on. I slack off in the winter because my feet are cold, but I will surely use it more often in the summer. It does make a difference but you must be patient.
