Q: What do I use to secure it

It fits securely on the trach. You don’t need something else to hold it in place.

Q: Does this fit a fr 8 inner cannula?

I use a CFS #8 shiley trache tube and it fits fine.

Q: Is there sizes for every tracheostomy ?

I bought by color. Purple or clear. Not sure

Q: Is this pmv 2001 packaged with one secure-it® that connects the valve to a tracheostomy tie to help prevent loss?

I am not familiar with that product but by your description that would be my guess. My wife wears one and when she coughs it will blow off. I tied a small electrical clip to it with clear 8 lb. fishing line and it works well. I can clip and unclip it easily when she sleeps. Just an FYI
